2010 Campers

Camp Manager - A big thank you to Michael Powell for another awesome year as our camp manager.  We are very fortunate that Michael has already volunteered to manage our camp again next year.

As always, with following VAD's standards, we will open the position of Camp Manager for anyone else who would like to be considered as well. If you are interested, please reply to VAD prior to our Sept. 28th, 2011 meeting and the board will consider all individuals interested.

Camper Application

1. To complete a Camper Application (Pages 1- 11), click the application link above.
2. Applications should be completed by entering information directly into the form using your computer.
3. Page 11 of the application contains a certification that all information provided is correct.
4. After you have clicked the certification button, click the submit button (upper right hand corner) to have the completed application emailed to VAD.
5. It is preferred that applications be sent via email. If unable to, they can be mailed via USPS by printing it and mailing to 28446 South River Road; Harrison Township, Michigan, 48045.
6. Campers must also complete the Camper Medical Form (Page 12).
7. Camper Applications must be completed by TBD.

Camper Medical Form

1. To complete a Camper Medical Form (Pages 12), click the form link above. This form is only required for campers.
2. This form must be printed out, taken to your primary care physician for completion.
3. Once your primary care physician completes the form, mail it to VAD at 28446 South River Road; Harrison Township, Michigan, 48045.
4. Signed, complete Medical Forms must be received prior to the start of camp.

Volunteer Application

1. To complete a Volunteer Application, click the application link above.
2. Applications should be completed by entering information directly into the form using your computer.
